VITAL STATISTICS
The Office of Vital Statistics certifies the records of marriage, birth and death
certificates that occurred in the City of Summit.
Birth, Death or Marriage Certificates
All requests for birth, death or marriage certificates must be either mailed to
the Summit Health Department or apply in person. Click here

ANIMAL LICENSING
The Summit Department of Health requires that all dogs and cats be licensed through
the City of Summit. The annual licensing period is from June 1 through May 31.
All renewal applications must be received by May 31 of each year.
